Cancellation Request Form

J memberships are initially for one year. Members who wish to cancel their membership after their first year may do so with 30 days written notice. Members are responsible for 1 additional month of membership dues following the termination request. If membership is cancelled prior to the completion of the first year, the remaining dues for the year will be charged. 

We always strive for member satisfaction and a positive member experience. We'd appreciate if you'd share with us your decision to cancel and any additional information that may help us better serve our members.

I intend to cancel my Siegel JCC membership. I understand that the Siegel JCC requires a minimum of 30 days' written notice of cancellation and that I am responsible for 1 additional month of membership dues following the termination request. I understand that at that time I will no longer be eligible to use the Siegel JCC facilities. I understand that I am responsible for all membership fees and charges on my account through the cancellation date. I also understand that by terminating my membership, I am forfeiting any and all fees paid to join the Siegel JCC and will be subject to the prevailing membership policies and rates, including all fees, if I decide to rejoin at a later date. I also understand that if I rejoin at a later date that membership priority date will begin a new period as of the date I rejoin. I agree to return my membership card(s) by the date of termination.
